Who We Are
Community Health Best Practices, LLC is a membership network of 17 of the nation’s leading federally qualified community health centers (FQHCs) partnering to improve the health of the communities we serve. All of our members are nonprofit, tax exempt (IRC §501(c)(3)) entities.
Brief History & Purpose
Community Health Best Practices, LLC is a nonprofit network comprised of 17 of the nation’s leading community health centers. Our Members operate federally qualified health centers (FQHCs) in Oregon, Washington, Arizona, California, Colorado, New Mexico, Texas, Hawaii, Connecticut, New York, Massachusetts, and Puerto Rico, and they provide comprehensive primary care and behavioral health services to all patients regardless of the patients’ ability to pay for such services.
Organizational Impact
The health centers that comprise Community Health Best Practices, LLC have a significant impact on the communities we serve. Not only are our safety net health care and social service programs vital to our patients and clients, but we are a major economic driver in many urban, rural and frontier communities throughout the United States.
